If the applicant is US citizen born abroad, a consular report may also be used as proof.
Aside from the applicant, his or her parents or guardians would also be required to submit certain documents. One of these documents should show proof of the child’s relationship with the parent or guardian. They should also provide an ID, as well as a photocopy of it.
As with other procedures dealing with minors, applying for a child passport also requires a written parental consent.
